I'd say they're a sandwich short of a picnic.

I don't mind those phrases as much as I dislike the inappropriate and all too frequent use of the word retarded which, by definition is only 2-3% of the population. And after having known a woman whose grandchild was born with anencephaly I don't particularly like when people say someone was born without a brain in a callous flippant way! MOST of the world is not stupid. MOST of the world is of average intelligence. And just because someone has a mental illness doesn't mean they're stupid. I have one and I'm nearly a genius, not bragging just stating the statistical truth. I'm no better or worse than anyone else and neither is anyone else!

I heard that same phrase all of the time growing up in Michigan. I'm fascinated by these sorts of idioms. "A few fries short of a Happy Meal" is one I've heard quite often, which is maybe a little more demeaning. In German, you'd hear someone told that they have a bird or that there are cups missing from their cupboard to mean the same thing. In Slovene, someone who is crazy would be said to be missing a gear in their head, or, oddly, that they do not score 100 points.
