They want: full name, mother's maiden name, birth date, social security # (or, at least: last 4 digits of social security #).

They could be asking all sorts of personal questions. If you never answer anything personal, they shouldn't be able to find you.

Personal questions gives identity thieves an opportunity to know who you are. They could ask about your name, DOB, address, social security number or other identifying numbers such as medical insurance or credit card accounts.
