If something's worse for wear, it has been used for a long time and, consequently, isn't in very good condition. A person worse for wear is usually drunk. If it is used in "not much worse for wear" It can mean not having become much worse as a result of the passage of time or frequent use. Such as "My car is old, but it still runs well. It's not much worse for wear."
