That is most likely a hot spot. He is itching unbearably. Usually, those are caused by an allergy. I had a friend who had been through three years of vet school, who swore they were caused by allergies to corn and soybeans. She said dogs weren't meant to eat those proteins and their bodies couldn't handle them. Whenever her dogs got them, she'd take them off dog food and give them butcher bones to chew the meat off instead. It always cleared them up. If you don't want the mess of butcher bones, try feeding them burger for a week or switch to another dog food that doesn't have corn or soybeans in it, like a lamb and rice dog food.
